How Can a Geriatric Care Manager Help?

Geriatric Care Managers can help seniors and their families in a number of different ways. Often, children need professional advise to help them help their parents.

1. A Geriatric Care Manager (GCM) can step in an evaluate the senior’s needs, such as what assistance she may need with activities of dialing living. Most seniors want to live as independently at home, but there can be a number of barriers which jeopardize their well being. For some families, there is no family member close in proximity to their parent to be on the ground in assessing their parent’s needs. Here, the GCM can be an invaluable resource.

2. A GCM can evaluate and advise as to housing options to maximize the senior’s quality of life. It may be that it is not in the best interests of the senior to continue to live at home. The GCM can assist in the evaluation process as to the options which may include assisted living or nursing home.

3. A GCM can assist in the transition from hospital to rehab to home. Often, seniors are thrown into crisis due to a catastrophic event. The senior and her family may well be in uncharted waters. The emotional trauma of seeing a loved one in a fragile state of being. The GCM with her knowledge and experience can help navigate these unchartered waters.

4. The GCM can be part of the team to ensure that a comprehensive plan is put in place for the senior. Often, the GCM will work closely with an elder law attorney so that government benefits can be accessed to assist in the payment of care; as well as ensuring that on-going decisions can be made in the best interests of the senior through the use of Advance Directives.
